How To Check The Consent Form Status?
To sign a consent form follow the steps given below.
First, search the patient from the Search Box situated on the top right corner or from the Patient List in Patients situated on the top left corner.
Click on Documents situated at the left side menu as shown below.
Then click on the Consent Form Status button as shown below.
Now you can see a separate chart of signed and unsigned consent forms
To sign a consent form click on the button Sign Consent Form as shown below.

To view the signed consent form click on the View icon as shown below.
To send a signed consent form to the patient portal click on the Send Icon as shown below.
