How To Send Consent Form To PurePORTAL?
To send a consent form to the patient portal follow the below steps.
First, search the patient from the Search box situated on the top right corner or from the Patient list in Patients situated on the top left corner.
Click on Documents situated at the left side menu as shown below.
Then click on the Consent Form Status button as shown below.
Now under the section “signed consent forms” you can view the consent form for each appointment.
Now click on the Send Consent Form icon to send a consent form to the patient portal as shown below.
To view the consent form click on the View Consent Form icon as shown below.
