How to Sign a Consent Form in PureEHR

How To Sign A Consent Form?

To sign a consent form follow the below-given steps

  1. First search the patient from the Search Box situated on the top right corner or from the Patient List in Patients situated on the top left corner.


  1. Click on Documents situated at the left side menu as shown below.


  1. Then click on the Consent Form Status button as shown below.



  1. Click on Sign and Lock for a particular appointment and consent.


     

  1. Now click on the “+”  button as shown below.



  1. Now scroll down and check the box written “I have read and given my consent” as shown below.

  1. Now sign in to the checkbox as shown below.

  1. Now click on the Save button situated at the bottom right side corner.

 .


  1. To view the signed consent form click on the View Consent Form icon as shown below.


  1. To send a signed consent form to the patient portal click on the Send Consent Form icon as shown below.
